Dear Client,

We are providing this worksheet to assist you in recording your contributions to various retirement plans.

This information becomes particularly important for state income tax purposes when distributions are taken. The amount you contributed under each of the categories is non-taxable for Massachusetts as well as some other states when distributed to you. However, without this worksheet completed we have no way of knowing what amount is to be treated as non-taxable.

A separate worksheet should be completed for each individual. Please do not combine information for married taxpayers.

You can get this data from your old tax returns or, alternately, off of statements from the bank or brokerage firm where the funds are invested.

To access the spreadsheet with Microsoft Excel, select this link, click the Save button, and save to your Desktop.  Once you minimize or close your web browser you can double-click the file on your desktop named IRA_Worksheet.xls and enter the information directly in the spreadsheet.  Once complete you may email the Excel file to amy@14churchstreet.com, or print and mail it to the address listed above.  To email the file after you’ve entered data and saved it, right-click the file on the desktop and select Send To, then Mail Recipient.
